Service Coordinator

I) Job Purpose
Each person receiving services in the HCS, TxHmL, and GR CFC in Tarrant County will have a single identified staff, called Service Coordinator (SC). The SC will assist the person in identifying and articulating his/her priority outcomes, personal goals, and hopes for the future as well as his/her requests and need for services and supports. The SC will elicit the person's feedback, coordinate, monitor the services for desires outcomes as identified in the Person Directed Plan (PDP).  A goal of Disability Services is to provide the people we serve with seamless consumer driven services and supports. The SC must also be knowledgeable about the array of services and supports available in the community.


II) Essential Functions
A. Under limited supervision, provides on-going services to assist persons with developmental disabilities or related conditions in developing a PDP, informed choice regarding Provider, and development of the initial Individual Plan of Care (IPC). This job requires use of initiative and independent judgment, aggressive intervention in the community, interaction and cooperation with other internal departments of MHMR of Tarrant County and providers.  This job also requires timely completion of state required documentation.

  • Participate in a discovery process with the person and others identified by the person which results in identification of personal outcomes or personal goals, barriers to achieving the goals, and identifying services and support to meet goals.
  • Coordinate/facilitate PDP process for each individual served with timelines established in Rules and Handbooks.
  • Develop narrative results of PDP for each individual served.
  • Assure that all processes and service planning respects the needs, requests, and preferences of the person and occur in a timely manner which are directed towards the achievement of the person's goals and desired outcomes.
  • Assure when barriers to achieving personal goals are identified, they are communicated to the person and service Provider.
  • Assist in the coordination and development of the IPC based on the results of the PDP for each individual served and a review/indication of approval or disapproval is noted in the Client Assignment and Registration System (CARE) database as outlines in the HCS/Department of Aging and Disability Services (DADS) Handbook.
  • Ensure that monitoring, service planning, and assessment is completed as specified in the HCS/SC Best Practice document.
  • Responsible for identifying community resources or natural supports that meets outcomes before recommending HCS.

 

B. Monitor the progress of desired outcomes. Schedule, facilitate planning/linking activities and conferences as needed to address issues and make revisions as indicated by need on the individual's PDP.

  • Monitor services provided to each individual monthly with how effective and appropriate current services are towards helping to meet his/her goals.
  • On a quarterly basis, complete a review of progress towards achieving each outcome identified in the PDP.
  • Elicit from each individual and ensure that revisions in the service plan are coordinated as the person's wants/needs change.
  • Ensure that updates in the PDP are completed in a timely manner and are based on consultation with the person and if appropriate, their family/guardian or provider.
  • Ensure that individuals are monitored with the frequency established by Best Practice or the Program Manager.

 

C. Complete all required documentation and ensure it meets Medicaid requirements.

  • Ensure that each individual has a current PDP/IPC.
  • Ensure that all required documentation is in each individual's chart.
  • Ensure timely of all documentation related to Administrative Claiming and Targeted Case Management.
  • Ensure timely completion/follow-up of all documents/requirements related to transfers and discharges.
  • Responsible for reviewing the Intellectual and Developmental Disabilities/RC (related condition) and indicating agreement or disagreement with the recommended Level of Need (LON). The SC is responsible for reviewing the Intellectual and Developmental Disabilities/RC according to the DADS Handbook Guidelines.

 

D. Build effective relationships with all stakeholders on behalf of the individuals served.

  • Ensure that Provider has a copy of the most current PDP.
  • Ensure that Providers are aware of concerns discovered when monitoring the person.
  • Assist in securing generic resources as needed.
  • Ensure that pertinent information is reported to the Program Manager/Assistant Director/Director as needed.
  • Ensure that all necessary/required information is forwarded to Providers in a timely manner.

 

E. Perform other job-related duties or responsibilities as requested or required whether or not specifically mentioned in job description.

  • Exhibits willingness to assume additional duties, seeks the guidance of supervisor prior to beginning an unfamiliar assignment.

  • Strong organizational/time management skills.
  • Knowledge of local community resources.
  • Ability to work in a non-judgmental manner with individuals whose values and belief systems may be different from staff.

 

IV) Internal & External Customer Service

Extensive internal and external contact is required. The employee will demonstrate advanced written and verbal skills while maintaining a positive image


V) Travel

This position requires driving between locations as an essential function of work. Work related training maybe at varied locations and employee is responsible for getting to and from the training location This position requires driving to and from locations for work and/and may require overnight traveling. Employees are required to have a license in good standing and be able to be covered by agency liability insurance. If personal vehicle is utilized for work travel, then maintaining personal auto insurance coverage is required.
