What You’ll Do
Evaluate patients to determine occupational therapy needs and develop individualized plans of care
Complete initial evaluations and ongoing reassessments in compliance with OASIS and PPS guidelines
Collaborate with physicians, caregivers, and interdisciplinary team members to ensure continuity of care
Implement patient-centered interventions focused on activities of daily living, adaptive equipment, sensory integration, and fine motor coordination
Educate patients and caregivers on home programs and therapy techniques
Assist with securing necessary equipment, including fabrication of custom splints when needed
Provide supervision and clinical direction to COTAs, HHAs, and other clinical staff as applicable
Maintain accurate, timely documentation in accordance with agency and regulatory standards
Physical & Work Requirements
Physically active, home-based work environment
Full range of motion, including lifting, transferring, and assisting patients
Ability to lift 50–100 pounds
Ability to travel within the assigned coverage area (up to 50%)
Potential for occasional irregular or on-call hours
What We’re Looking For
Graduate of an accredited Occupational Therapy program
Current, unrestricted Occupational Therapist license in the state of practice
Minimum of one (1) year of clinical experience preferred
Reliable transportation, valid driver’s license, and current auto insurance
Ability to sit, stand, bend, lift, and move intermittently throughout the day
You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee and be supported by a leadership team committed to your success.
