Direct Care Staff
Essential Responsibilities/Job Duties
- Learns and implements individual participant programs; consults with supervisor or other staff to clarify questions and/or understand program
- Implements programs for assigned participants; maintains contact with participants at all times; observes behavior; informs supervisors and other staff about participant issues
- Assists participants with activities of daily living
- Counsel participant to improve cognitive and social skills
- Assists participants who need assistance with standing, sitting, walking, etc.; lift and/or physically support participants when necessary
- Documents all required documentation, including, daily observations of participant activities, physical condition, accomplishments, etc.
- Prepares required reports
- Maintains and submits documentation in the format and timelines required by the Company, facility, local, state and federal regulations
- Shares information with supervisors and staff to promote communication and continuity of care
- Practice correct telephone answering protocol
- Meet and greet program visitors in friendly, professional manner
- Follow procedures regarding petty cash, receipts and timesheets
- Report needed repairs to appropriate supervisor
- Ensure safe, clean environment for participants
- Report unsafe conditions or activities
- Maintains a professional appearance, is reliable and punctual, cooperative and conducts self according to accepted standards of professional practice in the course of assigned duties
- Adheres to and participates in Company’s mandatory HIPAA privacy program/practices and Business Ethics and Compliance programs/practices
- Maintains the highest standards of professional conduct in relation to information that is confidential in nature. Shares information only when recipient’s right to access is clearly established and sharing of such information is in the best interest of the patient.
- Abides by and demonstrates the company’s Mission, Vision and Values through both behavior and job performance on a day-to-day basis.
- Performs other duties as assigned.
Qualifications
- High school diploma or equivalent required; further study preferred.
- Previous work experience, verifiable job history, and acceptable previous employment references
- Related work experience and/or training preferred
- Able to read and write in English
- Possesses current, valid state driver’s license and driving record meets company safe driving policy requirements
- Criminal background check results meet company and program requirements
- Demonstrates effective and professional interpersonal, verbal, and written communication skills
